Access to safe and reliable transport can help older people remain independent, maintain important relationships and stay involved in their community.

Dolare Care provides personalised community access and transport assistance based on each client’s mobility, preferences, routine and approved care plan. Our support workers can accompany clients throughout their outing where this forms part of the agreed service.

Services are delivered through Dolare Care’s approved Support at Home provider arrangement, in partnership with PlanCare ONEteam, and are based on your assessed needs, care plan and funding available through the Support at Home Program.

How We Can Support You

Depending on your assessed needs and approved services, assistance may include:

Attending medical, allied health and personal appointments
Grocery shopping, errands and carrying mobility aids
Visiting family, friends and community groups
Enjoying cafés, libraries, cultural and social activities
Planning outings and support throughout appointments or activities
Transport via an approved support worker’s vehicle, taxi or rideshare

Transport arrangements, availability and any applicable costs will be discussed and agreed uponbefore services commence.
